Pain and Symptom Management
Palliative Care focuses on comfort and quality of life with judicial use of pain and symptom management. Medications are explained and used wisely; time is taken for adequate pain and symptom assessment. Better pain and symptom control equals better functioning and a better life.
Palliative Care Planning and Management
Palliative Care is specialized medical care for people living with a serious or chronic illness. It is based on the needs of the patient, not on the patient's prognosis. It can be provided along with curative treatment. Identifying resources and strategies for meeting the needs of patients and families is central to the core of Palliative Care planning and management. It is the answer to the question - "What will I need, and how can I get it?"
Substance Use Disorder Management
On ML's Watch can help those persons affected by uncontrolled use of substances such as alcohol or illicit drugs. Medication-assisted therapies are provided with use of buprenorphine and naltrexone, for example. Special consideration is provided for those SUD patients also experiencing pain.
Chronic Disease
Care
Patients and families struggling with chronic illness have different needs which are unlikely to be met by standard care. Patients need regular and planned interactions with a focus on function and prevention of complications. This is what Palliative Care does.
End of Life Issues: Planning and Decision-Making
Making decisions for the future with discussion of Advance Directives, which are a set of instructions recognized under state law to provide healthcare when a person is incapacitated, is also what Palliative Care does for you. Everyone needs to know what you want!
Family Health Communications
High quality Palliative Care includes effective communication with the patient and their family and support systems. The patient's wishes are honored at all times. On ML's Watch offers the opportunity for family meetings and discussions, and shares information with other resources if allowed by the patient.
